The Clinicostatistical Study of Thyroglossal Duct Cyst

Abstract
The thyroglossal duct cyst is a relatively rare disease that was congenitally derived from the remnant of thyroglossal duct. It was found anywhere from the formen cecum to pyramidal lobe of thyroid. The author have observed the 62 cases who underwent operation for thyroglossal duct cyst at National Medical Center during the past 20 years from 1961 to 1980. The following results were obtained in the clinical and histopathological aspects. 1. The major age group of this disease was under the 10 years old, 37.1%(23 cases) was noticed under the 10 years old. The age of the onset of symptoms was less than 10 years old. The duration of illness, 37.1%(23 cases) was less than 1 year. There was no significant sex ration with 1.2:1. 2. The papable mass in the neck was major complaint that was noticed in 77.4%(48 cases) of all cases; the discharge from the sinus was noticed in 22.6%(14 cases), and other complaints were dysphagia, tenderness, but rare. 3. The location of thyroglossal duct cyst, 88.7%(55 cases) of all cases were found in midline of neck and others were found in lateral side of neck, 75.8%(47 cases) were situated at infrahyoid region, 17.7%(11 cases at suprahyoid, 4.8%(3 cases) at suprasternal, 1.1%(1 case) at intralingual region. 4. The presence of fistula, 43.5%(27 cases) were formed the fistula, and among the them 25.9%(7 cases) were made spontaneously, and 74.1%(20 cases) were made by artificially. 5. Histopathologically, the multiple accessory tract of cyst was noticed as 48.4%(20 cases), 12.2%(5 cases) had a single tract and 39%(16 cases) were impossible to detect the duct in the 41 cases that were preserved in availiable slides. The major lining epithelium of cyst and duct were composed of respiratory epithelium. The inflammatory reaction was found in 58.5%(24 cases), among them 62.5%(15 cases) was noticed at periductal area, and 37.5%(9 cases) was noticed around the cyst. The ectopic thyroid tissue was found in 14.6%(6 cases) and I case had a folicular adenoma. 6. The recurrence rate in 43 cases of Sistrunk¡¯s procedure was 4.6%(2 cases), and in 19 cases of simple removal of cyst was 21.1%(4 cases).
